The high prevalence of uncontrolled hypertension in Lower Salug Daku is a significant public health concern, driven by factors such as poor medication adherence, lack of awareness, unhealthy lifestyles, and limited healthcare access. A house-to-house screening revealed that a substantial portion of surveyed adults had elevated blood pressure, with a majority of diagnosed hypertensive individuals not effectively managing their condition. To address this issue, a comprehensive approach is needed, involving public health campaigns, regular blood pressure screenings and monitoring, patient education, lifestyle promotion, improved healthcare access, and appropriate treatment adjustments.

From the baseline

Out of the 200 adult individuals identified with elevated blood pressure readings:

  1. 107 (53.5%) are diagnosed with hypertension.
  2. 57 (28.5%) have no prior diagnosis of hypertension, with BP readings ≥140/90 mmHg on at least two occasions.
  3. 36 (18%) are pre-hypertensive.
  4. 100 (50%) fall under Stage 1 hypertension, while 64 (32%) fall under Stage 2.


Of the 164 diagnosed hypertensives:

  1. 51.22% (84) are compliant to medications, while 48.78% (80) are uncompliant.
  2. 2.44% (4) have a controlled blood pressure.

APPROACH TO

oBJECTIVE sETTING

The project aimed to establish a sustainable support system within the barangay through embracing a multifaceted approach that targets the enhancement of monitoring mechanisms, maintenance of medication supplies for hypertension, and promotion of healthy lifestyle through physical activities and health education.

general objective

To increase the hypertension control rate among diagnosed adults in Lower Salug Daku from 4% to 50% by July 2023.

SPECIFIC OBJECTIVES

  • To increase the proportion of hypertensive individuals with regular blood pressure monitoring from 0% to 60% by July 2023
  • To increase the compliance rate to antihypertensive medications among hypertensive individuals from 51% to 80% by July 2023
  • To increase the number of hypertensive individuals evaluated to have an active lifestyle to 20% by July 2023
  • To increase the number of community residents with adequate basic knowledge on hypertension by July 2023


Strategies & ACTIVITIES

Health Education and Promotion Campaign

  • Facilitation of health teachings during purok meetings and barangay-wide assemblies
  • Distribution of IEC pamphlets on hypertension among community residents
  • Facilitation of physical activity programs, such as the weekly Zumba sessions at the barangay hall and the modified sports for hypertensive males
  • Promotion of active lifestyle among adults through social media competitions (i.e., Zumba video performances)

Capacity Building

  • Establishment of the LSD Hypertension Club
  • Training for community health volunteers on proper blood pressure taking using manual sphygmomanometers

Development of

Public Policies

  • Signing of a Memorandum of Understanding between Barangay Lower Salug Daku and RHU Mahayag for easier access to maintenance medications among Hypertension Club members

Intersectoral Collaboration and Resource Mobilization

  • Distribution of Hypertension Club booklets among members
  • Collaboration with a local pharmacy (Jel Pharmacy) for the provision of discount cards to Hypertension Club Members
  • Lobbying of barangay officials for the stockpiling of emergency antihypertensive medications and BP apparatus at the health station
  • Facilitation of municipal-wide fundraising activities (i.e., Mahayag Fun Run) and partnership with local establishments

Physical Facilities

  • Procurement of BP apparatus for each purok
  • Facilitation of the distribution of signages for Purok BP checkup stations

Outcomes

As of July 2023, 98% (162/164) of diagnosed hypertensives had their BP monitored at the purok BP stations at least once a month. 98% were assessed to be compliant with medications; 66% (108/164) now adopt an active lifestyle. 97% of residents were evaluated to have adequate knowledge of hypertension. Overall, the hypertension control rate in the barangay significantly increased to 87%.
